Rear rail wear

So this is what I found today when I took out my rear suspension.
What would you guys do to fix this? Need ideas as I really don't have the money to replace the rails. I can't tell
how this started but I think it might have been a loose rear axle or tightening the axle down without the spacers
getting the cutouts correctly. Maybe just wear and tear?
Any ideas would be appreciated,
